Obesity is a major public health problem that has reached epidemic proportions in the US and around the world. Obesity is associated with significant morbidity and mortality. In recent years, our understanding of the mechanisms of obesity and novel therapeutic strategies for weight loss has expanded significantly. Advances have been made in our knowledge of energy balance, with respect to the hypothalamic circuitry controlling appetite and energy expenditure, specific fat types, and the role of gastric peptides, sirtuins, bile acids and gut microbiota in appetite, energy balance and regulation of glucose metabolism. This knowledge has led to novel treatment strategies for obesity as well as strategies that target visceral fat.
